Insurance & Fees

ABA Services (Billed to Insurance)

Evergreen Horizons LLC provides BCBA-led Applied Behavior Analysis (ABA) services that may be eligible for insurance coverage. When services are authorized, ABA is billed directly to insurance in accordance with your plan benefits, medical necessity, and payer requirements. Coverage and out-of-pocket costs vary by plan and may include copays, coinsurance, and deductibles. Families are responsible for any amounts not covered by insurance.

ABA services may include:

  • Initial intake and clinical assessment (as authorized)

  • Treatment planning and behavior intervention programming

  • Ongoing supervision and care coordination (as authorized)

  • Direct therapy services when applicable (as authorized)

Note: We will verify benefits when possible; however, final coverage decisions are determined by your insurance carrier.

Professional Services (Private Pay)

Professional services that are not billed to insurance are provided on a private-pay basis and billed at a standard rate of $125 per hour. These services are designed to support families with guidance, training, consultation, and communication needs across settings.

Professional services may include:

  • Caregiver coaching and parent consultation

  • School consultation and meeting support

  • Translation/interpreting support

  • Document review and written recommendations (non-insurance)

  • General consultation and coordination outside of insurance authorization

Advocacy Services (Membership-Based)

Advocacy support is offered through a 3-tier monthly membership model, structured as a time bank for 1:1 support. Membership services may include records review, planning support, meeting preparation, and participation in school meetings, depending on your tier.

Membership tiers and monthly rates:

  • Guided Membership — 60 minutes/month — $129

  • Advocate Membership — 120 minutes/month — $229

  • Partner Membership — 240 minutes/month — $429

If additional support is needed outside of the selected membership, options are discussed and approved in advance.

CEUs & Professional Development

Evergreen Horizons LLC provides continuing education and professional development trainings for educators and behavior-analytic professionals. Pricing varies based on training format, duration, and delivery method and is discussed during consultation or provided at the time of registration. Current offerings and details are shared with each training or event.
