Evergreen Horizons LLC provides BCBA-led Applied Behavior Analysis (ABA) services that may be eligible for insurance coverage. When services are authorized, ABA is billed directly to insurance in accordance with your plan benefits, medical necessity, and payer requirements. Coverage and out-of-pocket costs vary by plan and may include copays, coinsurance, and deductibles. Families are responsible for any amounts not covered by insurance.
ABA services may include:
Initial intake and clinical assessment (as authorized)
Treatment planning and behavior intervention programming
Ongoing supervision and care coordination (as authorized)
Direct therapy services when applicable (as authorized)
